Stoneham Medical Group (SMG), a Winchester Physician Associates practice located at 88 Montvale Avenue, Stoneham, has achieved Patient Centered Medical Home (PCMH) Level II Recognition through the National Committee for Quality Assurance (NCQA). PCMH is a model of care provided by medical practices that seeks to strengthen the doctor-patient relationship by replacing episodic care based on illness and patient complaints with coordinated care and team relationships with the patient. The physician-led care team is responsible for providing all the patient’s health care needs and, when needed, arranges for appropriate care with other qualified clinicians.
This Level-2 recognition (the second highest level) ensures patients and their family members that the practice is patient-centric, focusing on a patient’s entire care and considering them part of the healthcare team. SMG is WPA's second recognized practice. Winchester Family Physicians (WFP) achieved PCMH Level II recognition last year.
PCMH practices also emphasize enhanced care through open scheduling, expanded hours and communication between patients, clinicians and staff. PCMH focuses on many operational details, including policy and job description development, work redesign, implementing a patient portal, maximizing electronic health records usage within the practice, and involving the providers and care team in the initiative.
